Please call our office at (661)260-1282 to request medical records for another provider. We will need the provider's name, phone number, and fax number.
If a medical records request is for personal use, you will need to come into the office with your ID present and pay a $25 fee for your medical records to be released to you.
Please allow 15 days to process requests.
